Neither the Thule # TH963PRO or the Yakima SpareTime # Y02599 are height adjustable. However, either rack will allow you to shift the rack a bit to the side to center the bike better on the rear of the vehicle on vehicles with off-center rear tires like your RAV4. This could move the rack enough so that the bike didn't cover the license plate.

Even with a dual hitch extender like # MPG544, you would not be able to fit both a bike rack and a hitch mounted spare tire carrier into your hitch receiver. Also using a hitch extender will reduce your tongue weight capacity by half.

The Curt # D210 Multipurpose Ball Mount with 2-Inch Receiver is the item nearest to your requirements. This provides a 2-inch receiver on top, for use with a bike rack, and a ball mount at the bottom that will accept a ball with a 1-inch shank diameter, such as 1-7/8-inch # 19260 or 2-inch # 63845.
